The dynamic, multi-media curriculum, written in partnership with Creative Visions, is rooted in Jhpiego’s decades of expertise creating successful global health programs and provides high school students the opportunity to learn these methods through projects and problem-based learning.

“Changemakers teaches students about creative activism,” Brennan told the Hub earlier this year. “This course challenges young people to look at complex problems from many different angles. Students who learn about issues in international public health can channel their energy and enthusiasm to move society forward.”

Moreno’s hope is that students enrolled in Changemakers will come away with “an understanding that while nothing separates us as individuals in the U.S. from our sisters in less developed countries, in terms of mind, body and spirit, our access to quality health care may be vastly different from theirs.

“It’s our duty to understand those differences and, through the curriculum, we invite students to explore effective approaches to reducing that gap,” Moreno said. “We also encourage further study, as well as the pursuit of causes that most inspire them, leading to actionable change, such that one day that gap may be eliminated.”

Changemakers debut

The class was first piloted at Roland Park in 2020 as a 10th grade elective course for 13 students, many of them virtual due to the school’s hybrid learning format due to COVID-19. This year, Roland Park expanded the course, making it a year-long, requirement for the more than 60 9th-grade students. “I have been surprised, both this year and last, about the knowledge gap students have when it comes to women’s health topics,” says Sarah Morehead Pope, program coordinator for the Gore Leadership Institute at Roland Park, which oversees the Changemakers curriculum at the school. “We have a robust health and wellness curriculum at our school, yet we saw that our students are not well informed. Topics like HPV, the HPV vaccine and cervical cancer, maternal mortality rates and discrimination against women across the board in medical settings seemed to really strike a chord with our students.”

“I’ve learned a lot from the Changemakers class this year,” says 15-year-old Roland Park student Eryn Balchunas. “I never really thought that I could make a difference because I’m so young, but from this class, I’ve learned how people have made change in their communities, and that shows me that it’s possible to make a difference.”

Bringing challenging conversations home

The class hasn’t just been helpful for the students—it’s also created an opening for parents to have discussions about difficult topics with their daughters. “Eating disorders, anxiety disorders, women’s trafficking—these are not easy subjects for kids to bring up with their parents. Jhpiego also provided supervisory support, jointly with both federal and district EPI offices and relevant departments, during periodic visits of adult vaccination centers to ensure compliance with operating procedures, guidelines and data entry in the National Immunization Management System.

Monitors collected data from supportive supervision activities through the web-based mobile application KoBoCollect. They shared daily reports with federal and provincial EPI teams to watch for emerging issues such as human resource shortages, cold chain maintenance and proper waste disposal. The Federal EPI took prompt actions to mitigate problems. Monitors also shared and presented monthly progress reports with Federal EPI and partners. Monitors’ reporting compliance on KoBoCollect was outstanding: 96% of reports were received and analyzed.

Federal EPI staff take stock of COVAX vaccines at a federal cold-chain repository in Islamabad. Performance across all 10 indicators improved 17 percentage points; guidelines and trainings showed the biggest improvement with a 32-percentage-point gain to 90% at the final assessment in January 2022. Supportive supervision identified best practices, gaps and possible corrective measures and improved the understanding of staff on vaccine service delivery, including self-protection and safety of clients. On-the-job trainings improved cold chain maintenance, infection prevention and control, management of adverse effects following immunization and waste disposal.

The project team noted that supportive supervision by provincial monitors and provision of materials were instrumental in building the trust of facility staff. The study, taking place in South Africa, Lesotho and Tanzania, introduces two new strategies to test for latent TB (the presence of TB in the body, but without symptoms of illness). In 2018, with funding from The Global Fund to Fight AIDS, TB and Malaria, Jhpiego supported the Ministry of Health to establish a national TB contact tracing program under the THEBE (Prevention of HIV and TB in Lesotho through Evidence-Based Interventions and Education) project, which aimed to identify new cases through prompt and robust contact tracing strategies.

CUT-TB builds on these goals by adding latent TB testing for every household member of a TB index patient to Lesotho’s TB arsenal. Through a series of human-centered design workshops in Malawi and Zambia, “we realized that so much of the messaging that we create for men has shame hidden inside of it,” said Dotson. “People were telling us that ‘it takes courage to get tested’ because HIV is often portrayed as this scary, catastrophic disease.

“They weren’t getting tested because they didn’t feel like they had the courage to go down the entire path of managing a positive diagnosis,” Dotson added. “So we started thinking about how to break down courage into manageable steps: you just need to find the courage to do the next thing.”

Shortly after the RISE project’s launch in Nigeria, the implementing team saw a clear gap emerging between men’s and women’s use of HIV services. This approach required precise messaging that would resonate with men, building on what Dotson had heard in Malawi and Zambia. “For men, it is not enough to just increase risk perception and remove barriers,” said Adetosoye Adebanjo, the HIV testing Services (HTS) community advisor for RISE Nigeria. “Men have to desire the service and see how it will help them achieve what they most want.”

RISE put together a team of experts from Mann Global Health, Anova Health Institute and Jhpiego, including Dotson and Adebanjo. It’s all part of the South African government’s goal to make COVID vaccines more accessible to people living in vulnerable and outlying communities, like Khayelitsha and neighboring Mitchells Plain.

In Khayelitsha, just 37% of residents are registered to receive a vaccination, which means they’ve likely received at least one dose of Pfizer or Johnson & Johnson, two vaccines commonly available in South Africa. Nationwide, nearly 49% of the adult population has received at least one vaccine dose and Rosemary Njogu44% of the adult population is fully vaccinated.

With funding from the Solidarity Fund, Jhpiego has supported the South African National Department of Health to vaccinate South African residents in two provinces, the Western Cape – where Cape Town is located – and KwaZulu-Natal. As of May 9, Jhpiego has provided 40,378 vaccinations to residents in the Cape Town metro area. The project continues through June.

Building on VMMC work

As the pandemic reached South Africa, Jhpiego knew it could contribute to reducing the spread of COVID-19 through its experience in HIV prevention through voluntary medical male circumcision (VMMC). When the South African government asked for NGOs to assist with COVID vaccination campaigns, Jhpiego offered to help, drawing on its logistical, administrative and procurement experience with VMMC as well as its experience in generating interest in preventive care among adult men, who have been slower to take up the COVID-19 vaccine in South Africa, said Dr. Christopher Morgan, Jhpiego’s senior technical advisor for immunization. This included everything from understanding what materials would be needed – needles, gauze, sharps bins, etc. – and where to procure them, as well as how to answer questions that clients might ask, like, “Why can’t I stand for the vaccination? Why do I have to sit?”

In addition, because of its VMMC work, Jhpiego was able to draw on its strong relationship with the Department of Health and the communities it proposed working in for the vaccination program.

“We were already delivering a service, VMMC, and the health department knew they could trust us with vaccination,” said Dr. Cynthia Thandi Ncube, Jhpiego medical officer who helped coordinate the vaccination program with colleague Siyabulela Kwezi Shumi. “It was much easier to set up a vaccination site because of the experience we had from VMMC. We already knew what was needed and could be done.”

Jhpiego hired four nurses to run pop-up sites where they would provide COVID vaccinations and prepared for the work through the government’s online training course. Until recently, the on-the-ground vaccination teams also included community interns, who were funded by the South African government for six months. The interns focused on demand creation, registration of vaccination clients and sanitization of the work and waiting areas. Five teams of vaccinators covered a total of 56 vaccine sites during the month of April.

Ncube said the start of Jhpiego’s vaccination program required commitment and dedication. The number of people getting vaccinated was low and the vaccination team discussed ways to stay focused and support each other.

“I was listening to them and talking to them about how we can make this work better.” — Dr. Cynthia Thandi Ncube

Eventually, vaccination numbers picked up; as more and more people saw their friends, family members and neighbors getting the vaccination and staying healthy, “then they felt more comfortable getting vaccinated,” Ncube said.

Educating clients about COVID vaccines

Still, as in other countries, South African residents were hesitant about receiving vaccines for a number of reasons. A fear of needles. Concerns about vaccine ingredients. Plus a feeling of ambivalence about the vaccine since it doesn’t 100% prevent anyone from falling sick with COVID.

“The most challenging thing is . . . convincing people to get the vaccine,” said Magubane, one of the nurses. “Because you talk to a person, explain what’s going on about the vaccine but still they will say, ‘No, we will come back tomorrow.’ It’s a bit of a challenge.”

But Magubane wasn’t deterred. She says one of her approaches to overcoming vaccine hesitancy is to ask people to think about the health of the people around them, especially older or younger family members or people they come into contact with. She also shares her own story of getting vaccinated in May 2021 – back when only health care workers could access the vaccine in South Africa – and then falling sick with COVID in September 2021.

“I was diagnosed with COVID but for me it wasn’t severe,” she said. “When COVID started, I was working in a hospital. I saw in the hospital how severe it [COVID] could be. For me, it was like a normal flu. The vaccine for us health care workers, it really was a relief.”

Magubane’s most rewarding moments in this job have been when clients come back and say that after they received the vaccine, they caught COVID but they weren’t very sick because the vaccination helped them. She’s lost count of how many times people have shared this kind of experience with her.

“It’s like, wow, we did something good for another person,” she said. “Then that person can spread the word that the vaccine is actually helpful.”

Laura Elizabeth Pohl is a photographer-writer for Jhpiego based in South Africa.
