The doctor asked one question—what brings you to the clinic today?

“I told him it was my first pregnancy and that I was worried the baby wasn’t kicking. He told me to stop working hard and to relax. He scribbled on my clinic card and asked me to return after two months.”

Two months later, Purity started bleeding. “Something was wrong because I was only seven months. I called my husband, and we were rushed to the nearest health post, a private clinic. They refused to attend to me because they didn’t want any liability. They referred us to a public facility 10 km away. When we got there, it was too late to save my baby,” says Purity.

“It was the worst experience of our lives. We were waiting to hold a baby—instead we were wiping tears and asking lots of questions,” says Purity’s husband, Stephen Kyalo.

Stephen’s mother stepped in to help. “Mum asked us to come home so that she could take care of us and recover from the tragedy. She committed to walk with us,” explains Stephen.

Not long after, Purity was pregnant again. She attended all seven group meetings at the Wamunyu health center, one of the facilities in Machakos County, Kenya, that provides group antenatal care (G-ANC).

Wamunyu health center is one of 12 health facilities supported by Jhpiego with funding from the Bill & Melinda Gates Foundation. Staff in these facilities were inspired by the work of the Ekalakala Health Center, which found G-ANC to be a better way to provide care to pregnant women and re-organized its antenatal care services so G-ANC is available to all clients. Since early 2022, health care providers in all the facilities including Wamunyu have been trained and mentored to adopt G-ANC as the routine standard of care so they are better able to support mothers, like Purity, during pregnancy.

Purity sits with her daughter Claudia and her mother-in-law.

Jhpiego originally helped Machakos County introduce G-ANC in 2016-2017 as part of a study. Given the positive experience and the evidence generated from the study, the county health team expanded it into more facilities. In 2021, the Machakos community health management team partnered with Jhpiego to scale up G-ANC at public health facilities so all pregnant women were provided G-ANC. Jhpiego is introducing G-ANC in Amhara Region, Ethiopia, and also supporting scale up of G-ANC in Nasarawa State, Nigeria.

Rhoda Njeru, a technical officer with Jhpiego in Kenya, highlights the benefits of G-ANC for both women and health providers. “Group antenatal care is a simple intervention that helps women to learn and share experience through the journey of pregnancy with the guidance of a trained provider,” she says. “G-ANC has helped break the hierarchy of providers and women. They bond so easily and can reach out for assistance even when at home via a phone call, WhatsApp or a text message. Staff attitude has changed and they are friendly.”

Purity and her husband, Stephen Kyalo, pose with baby Claudia.

Group antenatal care is a high-impact intervention cutting across the continuum of care—from preconception, antenatal, intrapartum and postpartum period—giving assurance of a healthy baby and healthy mum.”

Studies have shown that G-ANC provides women with a positive experience of care. They learn more about maternal and newborn health, including how to prepare for childbirth. They form meaningful relationships and have better communication with their health care providers. As a result, they receive better quality care and return for more antental care visits over the course of their pregnancies.

Nurse Mercyline, who trained in G-ANC through Jhpiego and led Purity’s group, says, “We have open conversations because we are seated together. class="wp-block-paragraph">For two years, Joana Albino sought help for a growth in her left breast. She visited the local doctors a couple of times, but it didn’t help. Her condition worsened. “Because of the state I was in, I could not find work. I closed myself off and lived in my own world. Many things came to a halt,” says the 26-year-old, who lives on the outskirts of Maputo, Mozambique.

A visit to a Jhpiego-supported health fair at an armed forces barracks during breast cancer awareness month last October brought Joana the relief and referral she needed. “The nurses welcomed me, listened attentively to my history and screened me,” she recalls. This was her first encounter with health workers who gave her the support she needed. Suspecting cancer, the nurses referred Joana to José Macamo Hospital, where she received a diagnosis, treatment and hope for the future.

A recently released Lancet Commission report titled “Women, power, and cancer” puts many of the experiences faced by women like Joana at center stage. It highlights the discrimination and stigma faced by breast cancer patients who often defer seeking care. “This report compels us to look beyond the biomedical aspects of ‘women’s cancers’ and investigate the interplay of inequities like race, region, class, ethnicity, etc., that disadvantage women in multiple ways,” says Dr. Somesh Kumar, a member of the Lancet Commission who serves as Sr. Technical Director in Jhpiego’s Innovations office and as its India Country Director.

At Jhpiego, we have always adopted a women-centered approach to care, including our work in women’s cancers, and that is exactly what this report underpins—the need to increase equitable access to early detection and diagnosis of cancer, and co-create accessible and responsive health systems that provide respectful, quality cancer care for women.” — Dr. Somesh Kumar

The Lancet report states that of the 2.3 million women who die prematurely from cancer each year (which disproportionately affects low- and middle-income countries), 1.5 million deaths could be averted through primary prevention or early detection, while a further 800,000 deaths could be averted if all women, everywhere, could access optimal cancer care.

Joana Albino braiding her mother’s hair.

The World Health Organization (WHO) Global Breast Cancer Initiative recommends public education programs to encourage early detection, timely breast diagnosis and comprehensive breast cancer management.

Jhpiego’s breast cancer work, which is closely aligned with the WHO strategy, prioritizes early cancer detection for better treatment outcomes. Therefore, we are making efforts to bring advanced services closer to where they live, using innovations like AI-based ultrasounds and molecular diagnostics at the district level. We are ensuring that women have access to multiple forms of treatment and establishing strong patient navigation pathways that would provide them with support to complete their treatment journey. Through our partnerships, we are creating diverse investment opportunities and generating the evidence needed for scalability.”

When Joana reached José Macamo Hospital, the doctor determined that her left breast showed signs of ulceration and suggested removing all the affected parts and offered reconstruction surgery. Joana consented. The doctors performed her surgery in May. Today, Joana is continuing her treatment but has found strength in her recovery. “I feel like a happy woman, a complete woman. Today, I can say that I have regained my youth because I haven’t felt this way in a long time. Photo by Ester L. Hutabarat, MOMENTUM Private Healthcare Delivery.

Laili Harkita’s pregnancy was a tough one. When she finally gave birth at 36 weeks, her baby weighed just 1,900 grams, or four pounds. Jhpiego-trained midwives at Alia Hospital, a private maternity facility in South Jakarta, Indonesia, cared for the newborn, following strict infection prevention protocols. The midwives taught Laili to hold the infant skin-to-skin, similarly to how kangaroos hold their babies. Health workers from Indonesia to Côte d’Ivoire are teaching mothers and families to use this simple yet time-tested kangaroo care method to meet babies’ needs for warmth, breastfeeding, infection prevention, stimulation, safety and love. It also helps small and sick newborns gain weight and thrive.

Laili’s baby is no exception. One in every four babies globally is born too soon or too small, and every year, 2.4 million newborns die before they are a month old. Eighty percent of these deaths are babies weighing less than 2,500 grams at birth—referred to as low birthweight babies. Sub-Saharan Africa and parts of Asia account for most of these neonatal deaths.

“Though the world has made progress in reducing neonatal mortality, we still have a long way to go. If we want to reach global Sustainable Development Goal targets, we have to focus on small and sick newborn care, and continue pushing for quality essential newborn care, especially in fragile settings,” says pediatrician Dr. Bina Valsangkar, Jhpiego’s Principle Technical Advisor, Maternal Newborn Health, and a fellow of the American Academy of Pediatrics. “Two-thirds of small and sick newborns require special inpatient care, but not intensive care, to survive and thrive. Kangaroo mother care is an excellent foundation to build upon because it provides warmth, feeding and infection prevention, all in a nurturing environment,” she says.

Dr. Valsangkar asks a critical question in efforts to reduce the unacceptable loss of small and sick newborns: How can we prevent babies from being born too soon or too small? Unfortunately, there weren’t enough doctors available. This meant that as demand increased, so did the waiting lines. To lessen client wait time at health facilities, the VMMC Project worked with the Ministry of Health’s VMMC Coordination Unit to advocate for the health ministry and other government stakeholders to implement task-shifting, a solution that allows nurses to perform all tasks related to the VMMC procedure. The project convened monthly meetings with relevant government entities at the national and community levels, developed technical materials that summarized evidence for the safety of nurses conducting VMMC procedures, and shared plans around introduction, training and scale up.

On May 6, 2021, the Principal Secretary of the Ministry of Health granted approval for training and accrediting degree holder nurses to provide all tasks related to VMMC for HIV prevention.

The VMMC Project, in collaboration with the VMMC Coordination Unit, drafted a road map to operationalize the task shifting plans. Since then, the project has helped train 39 nurses from different facilities across all four regions in the country on the procedure and more than 16,500 clients have received VMMC.

Nurse Andile received her certification to be a VMMC nurse circumciser in February 2022.
